2017 NH Youth Risk Behavior Survey

On March 31st, JSRHS will be taking part in the 2017 Youth Risk Behavior Survey (YRBS) sponsored by the New Hampshire Departments of Education and Health and Human Services.  This survey is administered every 2 years and asks about the health behaviors of students in 9th through 12th grade.  This survey asks about nutrition, physical activity, injuries, and tobacco, alcohol and drug use.  It will also ask about sexual behaviors that could lead to pregnancy and sexually transmitted diseases. 

Students will be asked to fill out a survey that takes about 45 minutes to complete and it is scheduled to take place during advisory block.  This survey is completely voluntary, and the student's privacy is protected.  The results are intended to provide information that the school can use to address school health, and create programs that can help reduce risk behaviors.  If a parent would not like for their student to take this survey or a student does not wish to participate, please click the attachment below for the Passive Parental Permission Form that should be filled out prior to the survey date and returned to the School Counseling Center, so students can opt out and attend an alternative advisory, where they can work on schoolwork.  Students can still choose not to participate during the survey, but will be expected to remain in advisory.
